Performance Excellence Coordinator / Home Care

Full-time. Weekdays - generally 8am - 4:30pm. Flex hours as job responsibilities dictate. Weekends and holidays - administrative on-call rotation.

ProHealth Care, a regional critical and primary care provider that ranks among the most technologically and clinically advanced community health care systems in the country, seeks a Performance Excellence Coordinator / Home Care. This role is responsible for coordinating quality improvement, risk management, performance improvement and service excellence for ProHealth Home Care to increase patient satisfaction, clinical outcomes and improve the overall operational efficiency of ProHealth Home Care.

Bachelor's degree in related field or equivalent experience required. Registered Nurse licensure is preferred. Healthcare background required. Previous experience with process/quality improvement. Demonstrated skill in analyzing extracted data, making recommendations and implementing process/quality improvement initiatives. Experience with process/quality improvement techniques such as lean, six sigma, etc. Ability to think strategically and be proactive to anticipate and provide recommendations; advise at all levels; strategically address clinical practice gaps; develop productive and effective relationships across all levels; diagnose and successfully recommend and implement process improvement issues. PC proficient; literate in Microsoft Excel, Word, and PowerPoint. Strong communication skills with mentoring and coaching experience.

Strong knowledge of related regulations and compliance and industry-specific key performance indicators is preferred. Experience coordinating risk management/safety initiatives to improve quality and clinical experiences; conducting root cause analyses and implementing related plans preferred.
